Getting Ready for Your Visit

At the time of your appointment you will be asked to fill out forms, or you may print the forms directly from this site. At your visit, you will be asked for information regarding your current medical problem, past medical and surgical history, social history and family history. Specifically we will need the following:

• Any films (x-rays, MRI’s, CT’s), CD-ROMS, and the reports that pertain to the condition you are being seen for. You may bring the actual films or they may be on a disc. All of the films you bring to your visit will be returned to you at the end of your visit. 
• If you have undergone any other testing, such as EMG or nerve conduction studies
• Any reports of prior surgeries or procedures
• Insurance card or information, if it is a workmen’s compensation case or a motor vehicle accident
